Thank you, Davis-san. I see you’re starting a new medicine for diabetes today, so I’ll ask a few quick questions and I'll explain how to take it. Is that okay?

デイビスさん、ありがとうございます。今日から糖尿病の新しいお薬を始められるのですね。いくつか簡単な質問をし、飲み方を説明させていただきます。よろしいでしょうか。

Patient

Sure.

First, how have you been feeling lately? Do you have any fatigue or any symptoms that concern you?

まず、最近のご体調はいかがですか?倦怠感や気になる症状はありますか。

Patient

Nothing too serious. My blood sugar was high three months ago, so I was told to focus on diet and exercise. But I’ve been busy at work recently, and I haven’t been able to do it well. So I’m starting medication now.

それほど深刻なものはありません。3カ月前に血糖値が高かったので、食事と運動に注意するよう言われました。でも、最近は仕事が忙しくて、うまくできていません。それで投薬を始めることになりました。

Pharmacist

I see—you’ve been very busy. A lot of people go through that, and starting medication is a good step. Even small changes in diet and activity can help the medicine work better. By the way, may I ask your latest blood glucose and HbA1c values?

そうでしたか。とてもお忙しかったのですね。同じような経験をされる方は多いですし、投薬を始めるというのは良いステップです。食事や活動を少し変えるだけでも、薬の効果を高めることができます。ところで、最近の血糖値とHbA1cの値を伺ってもよろしいですか。

My fasting blood glucose is 140, and my HbA1c is 7.3.

空腹時血糖値は140、HbA1cは7.3です。

No. I’ve only had acute lower back pain.

All right. Your new medication is metformin. It helps lower blood sugar by improving your body's insulin sensitivity and reducing the amount of sugar your liver releases. Some people also notice a small amount of weight-loss benefit, especially when combined with healthy habits.

わかりました。新しいお薬はメトホルミンです。体のインスリン感受性を改善し、肝臓が放出する糖の量を減らすことで、血糖値を下げるのに役立ちます。また、特に健康的な習慣と組み合わせることで、若干の減量効果を感じられる方もいらっしゃいます。

Your directions are: one tablet twice a day, taken after breakfast and after dinner. You are receiving a 30-day supply of 60 tablets.

If you ever miss a dose, take it when you remember, unless it’s close to the next dose. If it’s close, skip it and continue as usual. Don’t double up.

飲み忘れた場合は、次の服用時間が迫っていない限り、思い出したらすぐにお飲みください。次の服用時間が迫っている場合は、飲み忘れた分を飛ばして、通常通りに服用を続けてください。2回分をまとめて飲むことはしないでください。

Pharmacist

The most common side effects—especially in the first couple of weeks—are stomach-related, like nausea, diarrhea, gas, or decreased appetite.

最も一般的な副作用は、特に最初の2週間では、吐き気、下痢、お腹の張り(ガス)、食欲低下などの胃腸に関する症状です。

Patient

I see. So, I'll watch out for my stomach at first.

そうですか。はじめのうちはおなかの様子に注意しておきます。

Pharmacist

Exactly. Also, if you have severe nausea, stomach pain, unusual shortness of breath, or unusually severe fatigue that feels different from normal, stop taking the medicine and contact your clinic or an emergency hospital immediately. The risk increases especially if you become dehydrated. During hot summer weather, when you can’t eat due to illness, or after intense exercise, please make sure to stay well hydrated.

その通りです。また、吐き気、胃痛、いつもとは違う息苦しさや強い倦怠感が生じた場合は、すぐに服用を中止してクリニックや救急病院にご連絡ください。特に脱水症状になるとこのリスクが高まります。夏の暑い時期や体調不良で食事が取れないとき、激しい運動をした後は、水分補給をしっかり行ってください。

Patient

Understood. I’ll watch out for diarrhea and nausea, and I’ll contact you right away if I have any severe symptoms.

わかりました。下痢や吐き気に注意して、もし何か強い症状が出た場合はすぐに連絡します。

Take one tablet twice a day after meals.食後に1日2回服用してくださいtake〜+ 回数 a dayで「〜を1日...回服用する」という意味。twice=「2回」/once=「1回」/〜 times=「〜回」(特に3回以上を表現する場合に用いられる)
